Resident Care Coordinator

Status: Full Time
Shift: To discuss during interview. Some weekends, holidays and on-call shifts may be required.
Wage: $22-$26/hr DOE
Location: Ovation Heartwood, 470 Maxwell Plaza, Omaha, NE 68154

Responsibilities:

  • Document information pertaining to changes in residents' functional status, personal needs, appointments, etc. and communicate changes to appropriate parties.
  • Maintain resident records according to the community's chart order.
  • Accurately measure, record, and assist with the monthly wellness clinic
  • Assist in the development and data entry of the service plan changes and oversee implementation.
  • File resident records and maintain discharge files.
  • Assist in scheduling appointments and coordinating care with outside services.
  • Conduct weekly, monthly and quarterly audits as outlined in the clinical standards.
  • Ensure proper documentation of all care plans, nurses notes and all corresponding documentation for the resident records according to regulations.
  • Assist in planning, conducting, and scheduling of in-service training classes, on the job training and orientation programs (in conjunction with Human Resources and Clinical Management) to ensure that current material and programs are continuously provided.
  • Assist Residents with activities of daily living in a way that promotes dignity and independence of each resident.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents.
  • Attend meetings of the Continuous Quality Improvement (CQI) Committee. Assist the CQI Committee in developing and implementing appropriate plans of action to correct identified quality deficiencies.
  • Ensure proper scheduling department working hours, personnel, work assignments, etc. as needed or required.
  • Assist in establishing the staff vacation scheduling as necessary or required.
  • Provide assistance as a caregiver or medication technician as needed or as directed by the Director of Health Services or Nurse on duty.
  • Attend Safety Committee Meetings. Create and maintain a safety culture. Be a good safety example, ensure safety expectations are communicated to all employees, ensures safety policies and procedures are followed.
  • While on the floor, assist with monitoring facility personnel ensuring they follow established ergonomic policies and procedures to prevent self-injury. Report all problems to the Executive Director.
  • Ensure that all personnel follow established policies governing the use/disposal of personal protective equipment and disposal of infectious waste.
  • Report any accident and incident reports such as falls, injuries of an unknown origin, med errors, abuse, etc. to Executive Director.
  • Ensure that adequate supplies and equipment are on hand to meet the day-to-day operational needs of the facility and residents.
  • Ensure that equipment/supplies are ordered/delivered on time and in accordance with established purchasing policies.

Qualifications:

  • Must possess High School Education or equivalent.
  • Must have experience as a CNA and/or medication technician for at least one year.
  • Resident Care Coordinator experience or similar preferred.
  • Must have experience with Excel, OnShift scheduling experience preferred.
