Hospice Bereavement Counselor

1. Provide coordinated patient care in an interdisciplinary hospice setting
2. Asses the patient/survivors emotional status of the patient and/or family. Asses the pre-bereavement status of patient/survivors
3. Develop and implement a patient goal oriented plan of care as a member of the clinical IDG
4. Assist and implement a survivor goal oriented plan of care for bereavement issues.
5. Develop, maintain, and contact as necessary, a resource group of clergy to whom specific aspects of spiritual care may be delegated.

6. Facilitate bereavement support groups

• Nevada drivers license and maintain personal auto insurance coverage.
• Must possess motor vehicle in good working condition that is available during working hours for job related tasks.
• Active license in the State of Nevada for social work, MFT, beavrement counseling, or related field and/or equivalent and relevant education and work experience
• Minimum one year of experience in the area of grief and loss and support group activities.
• Experience in program development and management.
• Experience in working with patients/families dealing with life threatening illness and death.
